How to Reset a Forgotten Username on Providence Mychart Account
If you have forgotten your username on your Providence MyChart account, you can follow these steps to retrieve it:
Step 1: Visit the Providence MyChart Login Page: Go to the Providence MyChart login page (https://mychartor.providence.org/mychart/Authentication/Login?).
Step 2: Find the “Forgot Username” Link: On the login page, look for the link that says “Forgot Username?” and click on it. However, if you do not remember your password, you may need to use the “Forgot password?” link first.
Step 3: Enter Required Information: You will be prompted to enter some personal information to verify your identity. Typically, this includes:
- First and Last Name
- Date of Birth
- Email Address or Phone Number
Step 4: Submit the Request: After entering the required information, click on the “Submit” button.
Step 5: Check Your Email: If the information you provided matches the information on file, you will receive an email containing your username.
Check your inbox (and spam/junk folder if necessary) for an email from Providence MyChart. If the email includes any additional instructions, follow them to ensure you can access your account.
Providence MyChart Login

For First-time users: Click the “Sign Up” button above the User Login form. A signup screen will display. You can either register using the email address that you provided to the office or you can register by creating a username and entering a security code provided to you from the office.
- Go to the Providence MyChart login page.
- Enter your Providence MyChart username and password in the spaces provided and
- Click Access MyChart.
